A doctor will need to complete this form if you are applying for a lorry or bus driving licence.

Details

 

Some doctors will be able to fill in both the vision and medical assessment sections of the report. If your doctor is unable to fully answer all the questions on the vision assessment, you must have it filled in by an optician or optometrist.

As of 20 June 2019, DVLA no longer accepts medical applications (D4) completed by Doctors on Wheels Ltd.

You cannot download the D2 application form because it includes features that will not print. You have to order a printed copy from DVLA.
